Trans input shaft failure?

Trans input shaft failure?

I have an 08 Fit with about 147K on it. About 10K ago the clutch and throwout bearing were replaced for the first time. Now there is apparently a transmission input shaft seal failure causing oil to leak onto the clutch (and slippage). Should this have been replaced when they did the clutch? As you might expect, dropping the trans and tearing it all apart again is getting overly pricey... quoted me $1500 this time.

It is a good idea to have the input shaft bearing and seal on hand when the trans is pulled to do the clutch as well as the rear main seal.
To replace the input shaft bearing and seal requires splitting the trans case which would add more labour time to the clutch job and wouldn't be done during a basic clutch replacement.

Update: apparently the input shaft bearings were obliterated and filled the case full of shrapnel, so their best option was to source a used transmission and they'll put a new clutch in it (one with 70K)...

What caused this failure? Misalignment during installation or incompetent removal? (some specialists are letting tranny to hang on the input shaft (but not sure if such thing is possible on FIT)
